Please review and complete the forms listed below, making sure to follow all of the instructions carefully. With the exception of the Medical Form, you will be able to complete the following forms on your own:
- 2021-2022 Participant Application
- Parent/Guardian Consent
- Photo Release & Consent Agreement
- Field Trip Permission Slip & Liability Waiver
- Electronics Policy & Agreement
- Photo of the Participant Please submit a copy.
- Covid-19 Vaccination Card Please submit a copy.
Please have all paperwork completed and returned as soon as possible. The deadline is 31 days prior to the start of your session.
